Can not get 2015 tax return info as of 12/2016 not yet filed. Tried for return of documents. No response to email phone calls, cannot ever fin him at home. What to do?

I am unable to reach Mr. Small after many tries via email and telephone calls. He has failed to reply. He has all my 2015 IRS paperwork....still waiting to file.
What do I do now? Does anyone know anything concerning his well-being?
