Mr.Melvin If you came to our Midfield office you would of received a copy of your 1040 pg 1 & pg 2 on the bottom of pg 2 you will find our firms information along with a phone number. If ever you feel an error has been made on my behalf or any other preparer you need to call that number. I apologize for any issues you might have had but i did not find your information in my system which means i did not file your return this year, last year or any other year prior too.
